The LPN will play a vital role in delivering personalized and client-centered healthcare, ensuring the well-being and comfort of our clients in collaboration with a multidisciplinary team.Responsibilities:Client Assessment and Care Planning:

• Conduct thorough assessments of clients' physical and mental health
• Collaborate with healthcare professionals to develop individualized care plans
• Monitor and update care plans as needed to address the evolving needs of clientsMedication Administration and Monitoring:
  • Administer medications as prescribed by healthcare providers
  • Monitor and document the effects of medications, making adjustments as necessary
  • Educate clients and their families on medication management and potential side effectsClinical Documentation:
    • Maintain accurate and up-to-date client records, ensuring compliance with regulatory standards
    • Document all interactions, observations, and interventions in a timely mannerCollaboration with Multidisciplinary Team:
      • Work closely with physicians, psychiatrists, social workers, and other healthcare professionals to coordinate comprehensive care
      • Participate in team meetings to discuss client progress, challenges, and adjustments to care plansClient and Family Education:
        • Provide education to clients and their families on health conditions, treatment plans, and self-care strategies
        • Foster a supportive and informative environment to empower clients in managing their healthQualifications:
          • Current and valid LPN license in the state of GA
          • Experience in nursing, preferably in behavioral health or developmental disabilities settings
          • Strong understanding of DBHDD regulations and guidelines
          • Excellent communication and interpersonal skills
          • Ability to work collaboratively within a multidisciplinary team
