If you are interested in joining the Allyon Team, please apply or submit your resume for review today!Job Title: Care Coordinator RN or SWDuties & Responsibilities: The Care Coordinator is responsible for managing and coordinating patient care across various healthcare settings to ensure that patients receive appropriate, timely, and effective care. This role involves working closely with patients, families, healthcare providers, and community resources to create and implement personalized care plans, address barriers to care, and facilitate smooth transitions between care settings. The Care Coordinator aims to enhance patient outcomes, improve quality of care, and optimize resource utilization. This is a 13 week assignment working 5 8 hour shifts with no weekends.Minimum Requirements:

• RN license or Social worker license (LMSW)
• 2 years of healthcare experience in the acute care setting-required
• background in medical social work in an acute setting-preferred
