Job Description:
AccentCare is seeking a dedicated Weekend Physical Therapist to
join our Home Health team, delivering exceptional care to patients
in their own homes. This role is pivotal in helping patients reach
their fullest potential while recuperating from illness, surgery,
or managing long-term conditions. The ideal candidate will have a
passion for patient care and a desire to work in a supportive team
environment. Our weekend therapists play a critical role in
ensuring continuous care, offering flexibility and personalized
interventions to enhance the quality of life for our patients.
Perform initial patient evaluations to determine mobility level and
pain, crafting effective care plans in conjunction with patient
goals and physician orders. Implement treatment plans utilizing
therapy techniques that may include exercises, stretching
maneuvers, hands-on therapy, and equipment to ease patients' pain
and increase their mobility. Regularly reassess patient progress
and adjust treatment plans accordingly. Educate patients and their
families on post-therapy care and lifestyle choices to ensure
sustained health. Maintain patient records, ensuring compliance
with legal requirements and clinical guidelines. Collaborate with
other healthcare professionals to tailor treatments and share
insights. Provide input to the continuous improvement of healthcare
services. Proficient in physical therapy techniques and tools
Ability to handle physical demands of the job Excellent

Insurance 401k Valid Physical Therapy license in the state of
practice CPR certified Valid driver's license and dependable
